Project overview
Systemic Improvement of Regional HealthCare for Resilient Peripheral Central Europe Regions
Peripheral regions face ageing populations, staff shortages and tight budgets that strain healthcare systems. CE-VITA aims to introduce a more efficient territorial care model, tested first on stroke rehabilitation. Patients continue therapy at home with digital support, reducing waiting lists and hospital pressure while keeping high-quality care. Health improvement hubs bring authorities, hospitals, NGOs and communities together to co-design and adapt the model and prepare a scalable blueprint for wider reform.
