For several pilot sites, this final workshop also served as a moment of celebration and closure. Participants and caregivers gathered to reflect on the progress made, the connections built, and the collective commitment shown over the past nine months. It was a time to recognize the dedication and shared efforts that brought the project to life.
We are now entering the final evaluation phase of our project. This critical stage involves in-depth assessments with senior participants to evaluate multiple aspects of the hybrid care model, such as the usability of the PROCAREFUL digital platform, their satisfaction with the workshops and the topics covered, and any potential changes in cognitive, physical, and social domains the project may have contributed to. To this end, a series of questionnaires will be administered to assess cognition, physical activity, loneliness, quality of life, and sleep quality. In addition, professionals and managers involved in the pilot activities will complete a questionnaire to assess the model’s effectiveness, its integration into existing service frameworks, and its long-term sustainability.
